As a Delta Mu Delta member, you can only be the recipient of a DMD Scholarship once per degree level (Bachelors, Masters, Doctoral).
Since you entered that you previously were the recipient of a scholarship for the same degree program, we are unable to proceed in excepting this application.
I have read the accompanying Applicant Instructions and accept the conditions put forth by the Scholarship Committee regarding these awards from the Governing Chapter of Delta Mu Delta. Additionally, I certify that to the best of my knowledge the information provided on this Scholarship Application is true and correct.
I understand that I must be or become a member of the Delta Mu Delta Honor Society no later than June 15 of the current year. This application may be submitted in anticipation of induction. Membership will be verified by the Central Office by June 22 of the current year.
